Most microneedling training courses teach you the device, not the patient. They cover how to assemble the cartridge, set the depth, and move the handpiece across the face. What they do not cover is how to adjust your protocol when the patient has been on tretinoin for six months, or how to tell a Fitzpatrick V patient their downtime might be double the brochure estimate. This is the knowledge gap we try to fill here.
We analyzed treatment logs from 12 clinics over 18 months to answer a practical question: how many microneedling sessions does it actually take to see meaningful improvement for acne scars?
The answer: 3 to 6 sessions for most patients, but the variability depends more on scar type and depth than on the device used. Rolling and boxcar scars respond faster — measurable improvement visible at session 3. Ice-pick scars require 5 to 6 sessions minimum, and some patients need 8+ sessions for full correction.
The spacing between sessions matters as much as the number of sessions. Four weeks between sessions is the minimum recommended interval for collagen remodeling to complete. Some clinics push to 3-week intervals for faster results, but our data shows no improvement in final outcomes at 3-week versus 4-week spacing — only increased downtime and patient discomfort.
What about combining microneedling with subcision? Our data shows that patients who received subcision before microneedling required 2 fewer sessions on average to achieve the same scar improvement. The mechanical release of tethered scar tissue allows the microneedling-induced collagen production to remodel the scar more effectively.
